Reporting Diagnosis Codes in ICD-10

Question:
My physician treated a patient for dysphasia secondary to an acute cerebral infarction in the inpatient rehab hospital. Do I need to report two diagnosis codes in ICD-10?

Answer:
You will only need to report one code. To report dysphasia secondary to an acute cerebral infarction you will reference the ICD-10-CM alphabetic index under “Dysphasia” which will further is defined as “following cerebral infarction I69.321”. The Tabular list should be referenced under category I69 (Sequelae of cerebrovascular disease) and further classified as I69.321. This is a combination code that allows for reporting of both conditions.
