Practice Assessment

This is our most requested service, with more than 1,500 practices nationwide engaging KZA to perform a 360° review of their businesses.

Consultations are customized to your particular practice and we review the following business systems:

  • Billing and reimbursement processes
  • Financial systems and controls
  • Operational efficiency and the use of automation
  • Practice management system and report generation
  • Human Resources policies and issues
  • Practice governance and business decision-making
  • Managed care contract administration

You will complete a comprehensive pre-consultation packet, followed by a site visit of two to four days. During the on-site portion we interview staff and physicians, conduct targeted operational observations, and review additional data. We share initial findings during a debrief meeting on the last day of the visit and keep in regular phone contact in the following four to six weeks to answer additional questions.

Reimbursement Process Analysis

In this service the consultant focuses strictly on the people, systems, and leadership that surround your accounts receivable process. The goal of this service is to decrease past due amounts and develop an efficient, effective receivables management system that prevents future problems.

You will complete a comprehensive survey packet about current reimbursement processes and sending us key reports and data prior to a two to four day on-site visit. During that on-site visit, the consultant will start by observing key systems and interviewing staff involved with the reimbursement systems, and end the consultation with a focused debrief session.

Practice Fees and Payor Rates Analysis

We can do either or both of the components of this service remotely.  It is an in-depth look at the practices charges and the payor rates.

Step One: Fee Schedule Analysis

Utilizing the RBRVS methodology, KZA will analyze your existing fee schedule and make recommendations to help you establish a fee schedule based on relativity and today’s reimbursement environment.  This is an essential process to maximize revenue without artificially inflating the accounts receivable. Our service begins when you send us your current fee schedule and a CPT frequency report. About two weeks later we will send you a full spreadsheet analysis for easy implementation, and a brief report that explains the methodology and highlights important issues.

Step Two (Optional): Contract Rates Comparison

Once your fee schedule has been thoroughly analyzed and re-established given the relativity-based standard for the industry, this analysis will show how your plan contractual payment rates compare to your fees, Medicare reimbursement, and other payor rates.

You send us the reimbursement rate data you’ve collected for your three biggest contracts, and we analyze and outline how those plans compare to your fees and Medicare rates. Knowing exactly how your current contractual rates compare to your fees and Medicare rates will help with budget decisions, practice growth strategies, contractual negotiation efforts and countless other practice management oversight and strategy efforts.

Coding and Documentation Audits

Depending on your concerns, KZA works with your practice to assess and address coding and documentation strengths and weaknesses. Each of our projects is customized to your practice goals with the data sample size mutually agreed upon. Depending on your needs, you can choose either one or both of the following services:

Evaluation and Management Note Review

This is an evaluation of how well your Evaluation and Management (E&M) coding matches its supporting documentation.

This process begins when you send us a CPT frequency report per physician. We then identify the specific visit type and level we want you to pull for our review. We review the documentation, code the note, then compare it to the practice’s code(s). We then write a report of findings, along with any teaching points.

Surgical Case Coding Review

This process begins in one of these two ways:

  1. You provide us with a CPT frequency report by physician, and KZA identifies the types of cases that have the most financial and/or risk management impact.
  2. You identify which types of cases are challenging you from a coding and/or reimbursement standpoint.

For each case, you send us the operative report, the claim submitted and the EOB received.  We provide an evaluation of how appropriately you coded the surgical cases, and how accurately third party payors have reimbursed your practice.
